What is Surgical Orthodontics?
Surgical orthodontics, also called orthognathic surgery, combines orthodontic treatment with corrective jaw surgery. This approach is used when the jaws themselves are significantly out of alignment — something that braces or aligners alone cannot correct in adults.
The surgery is performed by an oral and maxillofacial surgeon, with orthodontic treatment provided before and after surgery by Dr. Graf. This collaborative approach ensures optimal results for both function and aesthetics.
When is Surgical Orthodontics Needed?
Jaw surgery may be recommended for:
- Severe Overbite: When the lower jaw is significantly behind the upper jaw
- Severe Underbite: When the lower jaw protrudes beyond the upper jaw
- Open Bite: When the teeth don't meet properly when biting
- Facial Asymmetry: When the jaws are misaligned causing an uneven face
- TMJ Disorders: Jaw joint problems related to misalignment
- Sleep Apnea: When jaw position contributes to airway obstruction
- Difficulty Chewing or Speaking: Functional issues from jaw misalignment
The Treatment Process
1. Pre-Surgical Orthodontics
Braces are placed to align the teeth within each arch. This phase typically lasts 12-18 months and positions teeth for optimal results after surgery.
2. Surgical Planning
Advanced 3D imaging and planning ensure precise surgical movements. You'll meet with the oral surgeon to discuss the procedure in detail.
3. Jaw Surgery
The surgeon repositions the jaw(s) and secures them with tiny plates and screws. Surgery is performed through the mouth — no visible scars.
4. Post-Surgical Orthodontics
Braces remain for 6-12 months after surgery to fine-tune the bite and achieve the best possible result.
Benefits of Surgical Orthodontics
- Improved Function: Better chewing, speaking, and breathing
- Facial Balance: Corrects profile and facial symmetry
- Long-Term Stability: Addresses the root cause of the problem
- TMJ Relief: Can reduce or eliminate jaw joint pain
- Confidence: Life-changing improvements in appearance and function
Recovery & Results
Recovery varies by individual and the extent of surgery:
- Hospital Stay: Usually 1-2 nights
- Initial Recovery: 2-4 weeks off work/school
- Swelling: Peaks at 48-72 hours, improves over weeks
- Diet: Liquid diet initially, progressing to soft foods
- Full Healing: 9-12 months for complete bone healing
