What is a Palatal Expander?
A palatal expander (also called a rapid maxillary expander or RPE) is an orthodontic appliance used to widen the upper jaw. It works by gently separating the two halves of the palate, which are joined by a flexible cartilage suture in children and young teens.
This treatment is most effective before the palate fully fuses, typically before age 14-16, making early evaluation important.
Why Might My Child Need an Expander?
- Crossbite: When the upper jaw is narrower than the lower jaw, causing the back teeth to bite inside the lower teeth
- Crowding: Creating space for permanent teeth to erupt properly
- Impacted Teeth: Making room for teeth that are stuck and can't come in
- Breathing Issues: In selected cases, widening the nasal passages may help nasal airflow when directed by a sleep physician or ENT
- Bite Problems: Improving how the upper and lower teeth fit together
How Does It Work?
The expander is custom-made to fit your child's mouth and is attached to the upper back teeth. It has a small screw in the center that you'll turn with a special key — typically once or twice a day for several weeks.
Each turn creates gentle pressure that slowly widens the palate. After the desired expansion is achieved, the expander stays in place for several months to allow new bone to form and stabilize.
What to Expect
First Few Days
Some pressure and mild discomfort when turning. Speech may be slightly affected initially. These adjust quickly.
Gap Between Front Teeth
A space may appear between the front teeth. This is normal and often closes naturally or with the help of braces.
Eating
Soft foods for the first few days. Avoid sticky, hard, and chewy foods throughout treatment.
Cleaning
Food can collect around the expander. We'll teach you how to keep it clean with brushing and rinsing.
