What is a Herbst Appliance?

The Herbst appliance is a fixed orthodontic device used to correct significantly delayed or underdeveloped lower jaws and chins. Unlike removable appliances, the Herbst works 24/7 to guide jaw growth forward.

Decades of research have shown significant positive results in developing the lower jaw and chin for optimal esthetics and function. This results in a properly aligned bite and improvements to the profile, chin prominence, and overall lower-jaw development.

Who Needs a Herbst Appliance?

The Herbst appliance is typically recommended for children and teens with:

  • Significant overbite (Class II malocclusion)
  • Lower jaw or chin that is underdeveloped or positioned too far back
  • Upper teeth that protrude significantly
  • Cases where traditional braces alone won't correct the bite relationship

How Does It Work?

The Herbst appliance is attached to the molars with bands and has metal arms that connect the upper and lower jaws. When the mouth closes, the lower jaw is held in a forward position. Over time, this:

  • Stimulates growth of the lower jaw
  • Modifies the position of teeth in both jaws
  • Improves the bite relationship
  • Creates a more balanced facial profile and improved chin prominence

What to Expect

First Week

Some soreness and adjustment period. Eating soft foods and getting used to the appliance. Speech may be slightly affected.

Eating

Soft foods initially, then most regular foods. Avoid hard, sticky, and chewy items that could damage the appliance.

Sports & Activities

Normal activities can continue. A mouthguard is recommended for contact sports.

Cleaning

Extra attention needed when brushing around the appliance. We'll provide detailed instructions.

Advantages of the Herbst Appliance

  • Works 24/7: No compliance issues — it's always working
  • Effective: Proven results for overbite correction
  • Growth Guidance: Takes advantage of natural growth to support lower jaw and chin development
  • May Avoid Surgery: Can correct bite issues that might otherwise require jaw surgery in adulthood
  • Combined Treatment: Can be used along with braces for comprehensive treatment

Treatment Duration

Most patients wear the Herbst appliance for 9-12 months. Treatment is often combined with braces for complete bite correction and teeth straightening. The optimal time for treatment is during the growth spurt, typically ages 10-14.
